Required reporting
Reporting forms
- Illness Report Form (270 KB)
The owner or operator of a water recreation facility must report any illness associated with the facility to Public Health within 48 hours of becoming aware of such illness by calling 206-263-9566. Fill out this form and submit it to the email address provided on the form. - Injury Report Form (427 KB)
The owner or operator of a swimming pool or spa pool must report any death, drowning (fatal or non-fatal), or any other serious injury to Public Health within 48 hours of the incident by calling 206-263-9566. Fill out this form and submit it to the email address provided on the form. An investigation will be conducted for each incident.
