Do not upload these documents onto your organization’s website because it may lead to outdated versions being circulated.

Each year, a parent or guardian must authorize child care or early learning programs to use non-medical items by completing the following authorization forms:
Skin lotion or lip balm authorization
For non-medical products that are used to moisturize skin or lips (lotions, creams, ointments, or lip balms).
